Clinical and Scientific expertise

  • Calcium nephrolithiasis: physiopathology, genetic and nutritional predisposing factors.
  • Idiopathic hypercalciuria: physiopathology, calcium transport alterations, genetic and nutritional predisposing factors, clinical characteristics.
  • Hypercalcemias and hypocalcemias; physiology and regulation of cellular transport of calcium in human disorders; regulation of intestinal absorption of calcium.
  • Primary and secondary hyperparathyroidism.
  • Chronic kidney disease: conservative therapy, calcium and phosphate metabolism, hyperparathyroidism, metabolic bone disease and renal osteodystrophy.
  • Calcium-sensing receptor: function and role in physiopathology of human disorders, in calcium nephrolithiasis and hypercalciuria.
  • Obstructive nephropathy and retroperitoneal fibrosis.
  • Hypophosphatemia and hypophosphatemic rickets in adults.
  • Proximal and distal tubulopathies: Dent disease, Tubular acidosis, Bartter and Gitelman Syndrome, Cistinuria.

Main scientific works have been addressed to the study of nephrolithiasis and regulation of calcium excretion in humans. Findings reported in these works showed that polymorphisms of CASR gene, encoding for the calcium sensing receptor (CaSR), were involved in calcium kidney stone formation. Functional findings observed a reduction of the tubular expression of CaSR in carriers of these polymorphic variants of CASR gene. These studies also showed that variants of CASR and CLDN14 gene, this latter encoding for claudin-14, were involved in the regulation of tubular reabsorption of calcium and in idiopathic hypercalciuria. Moreover, these studies evidenced a faster intestinal absorption and tubular reabsorption of calcium after an oral load in hypercalciuric patients with kidney stones compared to hypercalciuric patients without kidney stones; these defects were associated with a decreased bone mass.
